It was a few minutes before 5. Dr. Sally Heddock, chief medical officer of Major City Hospital, just learned that noted staff surgeon Dr. Jim Cutler had almost left a scalpel inside yet another patient today. She knew the doctor wasn’t evil or mad. Rather, the problem was that he didn’t communicate well with his team either before, during or after surgery to make sure everyone was on the same page.
